I recently received an email from ASQ which may be viewed here. This features a FMEA by Thomas T. Reiley, M.D. As is common, this is actually an example of FRACAS not FMEA, since Dr. Reiley’s events are all existing errors. What’s bizarre about this is that I have an article that is linked on this very web page as some time ago I was invited to write an article for the ASQ healthcare site. This February 2007 article explains the difference between FMEA and FRACAS.
